W-9 Form
When a department or school is selling a good or service to another company, the company may ask for the »¨Ó°Ö±²¥ W-9 form and direct billing information so that »¨Ó°Ö±²¥ can be added to their system.
For W-9 requests from vendors, the W-9 for »¨Ó°Ö±²¥ can be accessed directly from Tax and Regulatory Services webpage (NetID log-in required). This form can be furnished directly to the requestor.
